Mike Durkin, NHS England's national clinical director for patient safety marks the tenth anniversary of the national reporting and learning system (NRLS).
A Health Foundation Shine 2012 project won the HSJ Patient Safety Award at last night's gala event. Great Ormond Street Hospital for Children Foundation Trust developed a new way of approaching patient safety that actively focused on service users as well as service providers.

The Health and Social Care Information Centre (HSCIC) has published the NHS Safety Thermometer report for October 2012 – October 2013, is a local improvement tool for measuring, monitoring, and analysing patient harms and 'harm free' care.
