This improvement report looks at the work of POPS (Proactive care of the Older Patient undergoing Surgery), a specialist multidisciplinary team based at Guys and St Thomas hospitals.

The team provide pre-operative assessment and optimisation for patients over the age of 65. Patients are followed through their surgical journey by the POPS team, who provides medical and multidisciplinary hands on care through to discharge from hospital.

This resource provides practical advice on pre-operative assessment and optimisation to help those looking at setting up similar models of care for older people who are undergoing surgery in their Trust.
