This evidence review was commissioned by executive nurses from Derbyshire and Nottinghamshire CCGs. It analyses the risks of elderly patients whilst in hospital including medication errors, delirium, falls, pressure sores, infections and frailty. It also looks at what might be done and how these models can be implemented.

The document is useful for all healthcare staff, particularly nursing staff in community and secondary care.
