This systematic review by Dr. Margaret MacAdam aimed to locate frameworks of integrated health care for for older people in Canada.

A substantial and growing body of knowledge has found promising indications that some models of integrated health and social care for the elderly can result in improved outcomes, patient satisfaction and cost savings.

This systematic review looks at the key elements of an integrated care framework for elderly patients and factors which strengthen the development of a pathway.

It reviews four frameworks, identifies congruence across the frameworks across most of their key elements and identifies four types of interventions that must be structured in ways that are supportive of each other.
