This How-to Guide from the Institute for Healthcare Improvement provides a step by step guide to identifying patients at risk of pressure ulcers and reliably implementing prevention strategies for all patients who are identified as being at risk.

It describes key evidence-based care components for preventing pressure ulcers, how to implement these interventions and recommends measures to gauge improvement.

The guide identifies six essential elements of pressure ulcer prevention and then goes on to outline tools to successfully creating and implementing prevention strategies. This includes such tools as a PDSA worksheet and suggested process and outcome measures.  

Multiple case studies from institutions in the US which have successfully implemented prevention strategies are also included.
