The Improving Prescribing for the Elderly (ImPE) Project

This improvement report documents the development of a medication review tool, based on the evidence based “STOPP” (screening tool of older persons’ potentially inappropriate prescriptions) for routine use with elderly patiens across all sections of care.

Adverse drug reactions are common in the elderly and it is estimated that around 35% of hospital admissions involving older people are due to inappropriate prescribing.

The project was funded by the NIHR CLAHRC for north west London.

Hi, I am a community matron currently looking at ways in which we can improve the way we undertake medication reviews. I enjoyed your article, and have searched the net for the full tool that you used (it states see overleaf for lists of meds, but not included in any articles I can see) Would you kindly forward me the full document to look at if this is possible.
