The incident decision tree provides straightforward and clear guidance tool to support a fair and just approach to patient safety incidents. The tool aims to support clinicians and managers in understanding when safety incidents should be attributed to systemic or organisational issues, as well as identifying the occasions when there may be individual culpability for an incident.

The purpose of the tool is to support building a just and fair safety culture that moves away from inappropriately blaming individual staff for safety incidents when these are more often the result a combination of human, organisational, technological and system factors.

The link to external website links to npsa website that has seemingly been out of use since 2004? Seems disappointing that this useful tool is not available to protect patients and staff in the care work.
