This report by the NPSA provides guidelines and suggested best practices for managing serious incidents that require investigation, including patient safety incidents.

The report focuses on the processes to be followed by healthcare organisations and staff, from notifying local risk management and patient safety representatives, to managing investigations and sharing learning widely throughout the NHS.

It establishes a standard definition of serious incidents and sets out the investigation, accountability and learning requirements of all organisations at different levels with the NHS when a serious incident occurs.
