Healthcare organisations are under increasing pressure to improve discharge processes and reduce readmissions - a key patient safety issue. 

Researchers at the Boston University Medical Center (BUMC) developed and tested the Re-Engineered Discharge (RED). They demonstrated that the RED was effective at reducing readmissions and posthospital emergency department (ED) visits and has since been developed by Agency for Healthcare Research and Quality (AHRQ). 

Although the RED was developed in the USA it is useful for healthcare professionals in all countries looking to improve the safety of patient discharge.
