This literature scoping review was commissioned by the National Patient Safety Agency (NPSA) as part of a project to determine any specific patient safety issues for vulnerable women known to be at higher risk of maternal death or morbidity.

This report describes the findings of the project to establish the patient safety themes for some vulnerable groups of women, highlight some issues and how they might contribute to patient safety incidents, and make recommendations for future work by the NPSA and others.  Issues identified included lack of access to care, lack of proactive support and fragmented services which are not culturally sensitive.
