The intervention was a care bundle for chronic kidney disease - a common complication of poorly controlled diabetes. The bundle included three evidence-based, high impact interventions based on National Institute for Care Excellence (NICE, 2008) guidance, with an additional and novel self-management element.

Patient activation interventions (PAIs) engage patients in care by promoting increased knowledge, confidence, and/or skills for disease self-management. However, little has been known about the impact of these interventions on a wide range of outcomes for adults with type 2 diabetes (DM2), or which of these interventions, if any, have the greatest impact on glycemic control.

For 12 months this programme sends personalised text messages to help people learn healthy eating habits, remember medications and appointments, and stay motivated.
