In the UK, Clinical Commissioning Groups (CCGs) and Health and Wellbeing Boards (HWBs) use clinical indicators to measure the quality of health services commissioned by CCGs and, as far as possible, the associated health outcomes.
The project by NHS Hammersmith and Fulham used co-production and developed from a diabetes service user group who wanted to provide more support and education for people with diabetes, and to raise awareness of diabetes in the local community. Volunteers were recruited to undertake champion, peer educator or peer mentor roles in Hammersmith and Fulham and Harrow.
A paper reports an experience of developing and implementing a whole-system, pathway of care driven approach to diabetes management across a health community over four years.
